New Patient Printable Forms
New patients can find health history and registration forms to the left, which should be printed, completed in pen, and taken to the first visit. These forms are also available to be filled out in our dental office during your first visit. They must be signed and returned before we can begin dental treatment.
If you have dental insurance, please include on the forms your group number and the subscriber's birth date and social security number, so we may assist you in maximizing your benefits.

Appointment Protocol
We would like to take this opportunity to explain our appointment protocol. When patients do not show for appointments, or cancel an appointment on short notice, many things happen. First, your treatment is delayed, which in some cases can further complicate your condition. Second, another patient waiting for care cannot be seen because we didn't have sufficient time to make the necessary arrangements to see them. Also, the doctor, or hygienist will have to wait until their next scheduled patient arrives for them to resume their work.
We do understand that occasionally situations may arise which require you to reschedule your appointment with us. However, due to the reasons listed above, we do ask that you inform us of any necessary changes within 3 business days of your appointment.

We belong to Envirostars - A regional pollution prevention recognition program provided by local agencies in King, Kitsap, and Snohomish counties in parternship with the Puget Soundkeeper Alliance. Envirostars has given our office four stars.

Silverdale Dental Center is proud to say we use The Amalgam Collector™. The Amalgam Collector™ is designed to trap the wastewater generated in dental procedures. Heavy metal particles and other solids are separated out, and the remaining filtered water and other fluids are released into the sewer system. This helps lower the burden of the silver, zinc, copper and bound mercury that reach our water treatment plants, so that our drinking water and our environment contain a lower percent of heavy metals.
